Joel Bloomberg Hand Blown Table Lamp Circa 1998. This two bulb lamp has lustrous blue iridescence at the right angles, and beautiful colors throughout. There is a second small bulb in the lamp base.

The area around the finial hole on the top of the shade is rough and seems chipped, but I think is original as it's quite precise looking, see picture #7. It's also a very thick shade and not a lot of finial thread exposed for a tight fit, so I don't use the included washer. Two small scratches on inside of the shade as shown in picture #8, not cracks but scratches.
