"Style" Pond Lily Stained Slag Glass Table Lamp Floral. Base appears to be a marriage made by Leviton. I didn't see any makers marks on the shade. Very beautiful lamp with mint blemishes as pictured. Some discoloration and patina loss in a couple spots. Please feel free to request additional pics or ask any questions. It's an exquisite Lamp either way. The cord is frayed and will need rewired eventually but it still works, as pictured. In the Lighted pics, I only have 2 out of the 6 bulbs inside.
